WIC promotes healthy mothers, infants, and children by providing nutrition education and supplemental food coupons to pregnant and postpartum women, infants, and children up to age 5. Participants must meet financial guidelines (click to view), and medical or nutritional risks. WIC provides breastfeeding support and vouchers for purchasing nutritious foods, such as formula, milk, cheese, eggs, fortified cereal, and peanut butter or dried beans or peas. Clinic locations:

In order for the WIC staff to have your coupons and paperwork prepared, an appointment must be made for the clinic you wish to attend. Our usual office hours are 8-12 and 1-5pm.  If  routine office hours are difficult for you, ask the appointment scheduler about our late and early clinics. Most new WIC clients will be given an appointment at one of our main clinic locations  in Stanton, Ithaca, or St. John’s.  There are convenient locations available for future WIC visits. WIC Appointment Checklist:

PLEASE BRING THE FOLLOWING ITEMS TO YOUR APPOINTMENT:

  1. Proof of identity (Example:  Drivers license or utility bill)

  2. Proof of income (Household) or Medicaid card

  3. Proof of residency (Example:  Drivers License or utility bill)

  4. The actual child or infant you are enrolling

  5. Verification of newborn birth (Example:  birth record, discharge papers or crib card)

  6. Immunization records for child(ren)

  7. Proof of pregnancy

  8. Court documents (guardianship papers) if foster parent or grandparent

  9. Paperwork you may have from being on WIC before

  10. Completed WIC health history form(s)

BE SURE TO BE ON TIME FOR YOUR APPOINTMENT!

If you are late and/or don’t have the above items your appointment will be rescheduled.

PLEASE CONSIDER BREASTFEEDING YOUR BABY; WIC CANNOT SUPPLY ALL OF YOUR BABY’S FORMULA. WE WILL NO LONGER HAVE EXTRA FORMULA IN THE OFFICE.
