Diabetes Outpatient Program

The Diabetes Outpatient Program of Montcalm County is a collaborative educational program involving all four county hospitals and the health department:

  • Carson Health Network

  • Kelsey Memorial Health Center

  • Sheridan Community Hospital

  • United Memorial Health Center

  • Mid-Michigan District Health Department

Diabetes education is provided with a team approach to address the individual's needs and/or family in an outpatient setting throughout Montcalm County. Education includes promoting self-management, blood glucose control and prevention of acute care and long-term complications. Staff includes certified diabetes educators, registered nurses and dietitians. A fee is charged for educational services. (Note: Many insurance companies cover some or all the sessions, if ordered by physician.)

Mid-Michigan District Health Department does not offer any classes, but patients may attend classes at any of the four hospitals listed above.  For further information, please call
